BronxWorks helps individuals and families improve their economic and social well-being. From toddlers to seniors, we feed, shelter, teach, and support our neighbors to build a stronger community. BronxWorks has provided homeless outreach in the Bronx via contracts with the NYC Department of Homeless Services since 1995. Our outreach team provides mobile case management services to a caseload of over 400 chronically street homeless adults, with the ultimate goal of ending their homelessness. The Clinical Coordinator directly supervises the team’s case management staff and works with the Program Director to administer and sustain the program.
