Nationwide Therapy Group is seeking a Skilled Nursing Facility (SNF) LPN / LVN for a travel job in Bristol,, South Dakota.

• Shift: 11.5-Hour Nights
  • Hours: 6:00 p.m. – 6:00 a.m.
  • Schedule: Every Other Weekend (EOW)
  • Guaranteed Hours: 36/week
  • Contract Length: 88 Weeks
• Facility: Sun Dial Manor
  • Type: 37-bed Skilled Nursing Facility with Assisted Living (Johnson Center)
  • Location: [Not specified—assumed South Dakota]
• Orientation: Shadow full shift; can start any day of the week

• Dress Code:

  • Professional scrubs (any color)
  • No other facility logos
  • Badge worn at chest level
  • No acrylic nails; professional piercings only
  • Tattoos and vibrant hair color are permitted

   

  Unit Information:

• Wings: East, South, West (central nurses station)
• Average Census: 27–30 residents
• Charting System: Point Click Care
• Communication: Walkie-talkies
• Medication Delivery: Bubble pack med carts
• Night Staffing:
  • Before 10:00 p.m.: 1 RN or LPN + 2 CNAs
  • After 10:00 p.m.: 1 RN + 1 CNA

• Floating: To Assisted Living as needed (med pass)

   

  Required Experience & Qualifications:

• License: Active LPN License in South Dakota or Compact State
• Experience: Minimum 2 years in Long-Term Care

• Certifications:

  • BLS (American Heart Association only)

  • Full 7-year work history (must list facilities, not agencies)
• Skills Checklist:
  • Completed within 1 year for the submitted specialty
• References:
  • Two (2) required from last 3 years
  • At least one (1) from a supervisor
• License Verification:
  • Online verification (e.g., Nursys), verified within 30 days

• Background Check (within 30 days of start):

  • National Criminal Check
  • FACIS Level III
  • National Sex Offender Registry
  • All counties lived/worked in last 7 years
  • SSN Address Trace
  • OIG (Office of Inspector General)
  • SAM (System for Award Management)

   

  Health & Compliance Requirements (Onboarding):

• 10-Panel Drug Screen: Collected within 60 days (required annually)
• Physical Exam: Within 1 year (signed by MD, DO, DC, NP, or PA)

• Immunizations:

  • MMR: 2 doses or positive IGG titer
  • Varicella: 2 doses, positive titer, or provider-confirmed history
  • Hepatitis B: Vaccination series, positive titer, or declination
  • Influenza (Oct–Mar): Declination accepted
  • Tdap: Within 10 years
  • COVID-19 Vaccine: Proof or medical/religious exemption
  • TB: Two-step PPD, Quantiferon, or T-Spot within 1 year
    • Positive PPD requires Chest X-ray (valid 5 years) + annual TB questionnaire

   

  Competency & Documentation (within 1 year):

• Competency Exam (80% minimum)
• Joint Commission Modules (Core I, II, III – 80% minimum)
• Government-issued photo ID (Driver’s License or equivalent)
